Reflow syndrome is a concern in just plain old bondage without suspension. You need to keep an eye on the bottom and make sure blood flow isn't being cut off to the limbs. If a significant amount of toxins and lack of oxygen have been built up by the bondage, then releasing the bondage and allowing said blood to race back to the heart can have adverse effects the most extreme being death. http://www.suspensiontrauma.info/introduction/reflow/ "When all the oxygen is used up, the cells start to burn fats.
