Greetings That depends on how it is done, but yes there is a chance of scars, but it is also many who do it without much scarring. Remember a nail is rather thin, it do not do allot of tearing of flesh, it is like a thick needle and if it is fairly sharp the wounds should close without allot of scars. However off course with things like this you have to learn to do it properly so everyone stay safe. I have never done it, but I have seen videos and there are generally not huge, open wounds left, the wounds pull themselves together quickly, again depending on how you do it, I would not let just anyone put a nail trough any part of my anatomy.
