Ad block doesn't block the picture itself, or even the object on the page. They block the server it's coming from. There are 3 reasons I know of that will cause them to stop working: 1) They change the server, poof, unblocked. I'm sure many do just that to bypass adblock software specifically. Others may sell their advertisements through multiple "networks" so the same ad will be delivered through different companies. Any not in your block list will get through. 2) They deliver it via a script that is part of the site. Sometimes adblock software won't work for this at all, other times it will work but they can update the scrip to defeat it. This turns into a cat and mouse game where the advertisers break the block and the adblock companies work to resume blocking. 3) The blocking software itself stops working. Crashed, user disabled it, an update broke it, etc.
