I understand your problem, am experienced with guys who have had this and have two comments to make. First, if your testicles have shrunk, get yourself to a doctor pronto and have your testosterone level checked. My boy had this problem because of a mining cave in and being on pain meds afterward. A CB wouldn't stay on because his balls were too teeny to hold up the cage. He's on testosterone shots now. (The CB was something his previous owner had him order. He doesn't need it with me. My voice and his need to submit to my will are enough. Putting one on him would be hawt for a scene though, I'll concede that much.)
Two. From one obese person to another, you're probably not going to like hearing this. I have topped other obese males and some of them have so much fat at their pubic bone that it bulges out around the penis and sac and...the guys end up looking a bit like a girl cuz their dangly bits get sucked up inside...unless they have a boner. Again, see your doctor. If your balls cannot hang or pull tight to your body, a constant warmer temperature might goof up your fertility.
There are other chastity devices to choose from, some that are not a cage but like one a woman would wear. [;)]
