Yes, a cold reading is one in which you're given the script and then told to act it out. A head shot is a picture of you (generally a fairly close up face shot) in the size specified. Oftentimes you will attach (with a paper clip) a card with your statistics on it; age, height, weight, hair colour, eye colour, age-range you're able to play. The best thing to do is to call ahead and ask them exactly what they're looking for and what info you should include. ETA: after researching a bit, industry standard is 8 by 10.
