As noted above, for most it means 'drug & disease' free. The problem is that "DD" has several meanings in this community - sometimes it refers to a bra size, sometimes it refers to "domestic discipline," and I wouldn't be shocked to find a sizable minority using "D&D" to refer to the "Dungeons & Dragons" role playing game. So watch for the ampersand, and context, and ALWAYS ask if you don't think the usage is clear.
