I have done some post-ejaculation CBT, but lightly. As you point out, it tends to be quite sensitive, so I got the reaction I was looking for very easily.
From the bit of research I've done, the risks of CBT tend to be on the micro-scale, and cumulative over time, although of course there can be severe trauma if something goes seriously wrong. You can still be damaged even if it doesn't hurt, and even if something hurts, it might not be risky at all. For example, I've had a couple of normally heavy masochists yellow from me pinching their thighs just below their butt. If I'd kept going, or done it a bit harder, it still wouldn't have caused any serious or permanent damage, just minor bruising at most. It's a sensitive spot, so it gets a big reaction. I bonked my head pretty hard when I was bottoming during a scene, and it didn't hurt at all at the time. I had a knot on my head, and it was really sore the next day, and tender for about a week. If I'd smacked it hard enough to get a concussion, the fact that it didn't hurt at the time wouldn't have reduced the damage. If someone accidentally breaks a bone or intentionally gets cut with knife play, their endorphin levels don't change the physical damage done.
