We had that problem, my wrist is so small that although metal handcuffs are ok, commercial leather ones wouldn't tighten anywhere near enough. I made my own from a set of these velcro hanging straps. http://www.directa.co.uk/site/scripts/product_browse.php?product_id=3162 It's like a broad webbing strap with loop velcro along most of one side, and a few inches of hook velcro at one end of the other side. I cut them to size, which involved cutting off the hook velcro end, sewed the hook velcro on at the new end, and bingo. There's a strong built in metal ring at the other end of each strap, it's hidden by the black plastic thing in the photos, but that just pulls off, and even once you've threaded the strap through the ring when fastening them, there's plenty of room to attach rope or snaphooks. They're both comfortable, and secure.
