I don't think it's necessarily the length, more the girth, that can become the problem. If she knows how to suppress her gag reflex then the length shouldn't matter whether it's 4 or 10 inches. But if it's too wide to fit down her throat, well it's just not going to go any further. Like trying to stuff a beer can into a toilet paper roll. Nice visual, I know! :P

Yes. And without numbing spray (which is dangerous, not to mention cheating). Personally, I can only do it if I'm controlling the descent and my ability to breathe. Otherwise, there will be unpleasantries.

I am not able to get past gagging, but that is due to a small mouth and bone malformation. It also causes my TMJ to act up. If the guy has a wee willie, I am fine.
