I've had my nipple piercings for nearly 10 years, so I actually had them before I tried anything like clamps on my nipples. I have smallish nipples and I wear curved barbells in mine. On the rare occasion that I have had nipple clamps on, they were always placed behind the piercing. I've also had to remove mine for surgery in the past, and they were out for nearly 8 hours without closing up, but I've always been lucky in that respect. I leave my labret piercing out for weeks without it closing up.
I do know that a lot of people tend to have bodies that "reject" the piercing or constant fluid leakage, so I'm not sure if that could make the clamps more of a hazard. I'd check out the BME site, there's an incredible amount of information there.
