lasceration advice needed

Status: offline 		hey all.   yesterday I lost my balance which slammed my hand into a jagged glass on a replacement house window I was attempting to install.

Seeing the cut right away I knew it was not the minor same old variety. So I went to ER, got 3 stitches, however they were very busy, I left with out it being dressed. [she had to have a written dr order, so I grabbed some band aids and that was that]

Anyhow I did not dress it as that was harder then I thought. It now is puffy, it had been straight on. Today tho it is swollen tho not as hot to the touch as yesterday. 

I have put ice on it. I do have some extra tetracycline, took one.  I think I have some medicated cream.

Anyhow- tomorrow I will call my dr.  How do I tell how bad the cur is?  The knuckle is where the tear is.

Status: offline 		you should have it checked for infections, see the doctor immediately.  put hydrogen peroxide around the affected area in the meantime.

Get your butt back to the ER pronto.

NEVER leave the ER with an undressed wound.  Let them finish the job they started.

Swelling and heat are indicators of infection -- and your extra tetracycline may not be enough to combat it. 

Worst case you encounter something lovely like MRSA.

Go back to the ER and get that cut dressed properly and get a proper regimen of antibiotics for the infection--and take the ENTIRE regimen.

(side note: "extra" antibiotics means you're not taking the full regimen.  That's a bad habit you need to lose post haste).

Status: offline 		NEVER put full strength hydrogen peroxide on an open wound.  It is too harsh for tissue.  All antibiotics are not created equal and tetracycline isnt the drug of choice for that kind of injury.  You need Keflex.  Until you can follow up, and any urgent care center can take care of your problem now that you have stitches in, use some brand of triple antibiotic ointment and keep it clean and covered.  You need a wound check after a couple days anyway.  Joint lacerations take the longest to heal because of constant flexion/tension.  You dont want to end up with something nasty growing in there.  And dont take the sutures out yourself when its time.

You need to see a doctor ASAP.  You are describing symptoms of infection.  How many Emergency Rooms are in your city?  Do you have a Urgent Care type of facility available.  Do Not Wait until tomorrow!
 
Please let us know what happens after you are treated by the doctor.

and next time put something about tending wounds in the title.  i thought you wanted advice on the proper way to llacerate someone else.  Got my hopes up and all.

Status: offline 		don't screw around, with the prevalence of mrsa these days, you could end up losing a limb.  get back to the ER and get that thing tended to properly!  keflex is the drug you're going to need, it's one of the best for treating lacerations and other skin infections known as cellulitis.  let us know how you're doing when you get back from the doctor.

Status: offline 		Ya know, popping antibiotics that you happened to have lying around the house ("you" in a general sense) instead of taking a whole regime of PRESCRIBED antibiotics is the reason we now have mutated bugs like MRSA these days.  STOP DOING THAT!!!

You'd have been just fine to apply antibiotic ointment and a dressing when you got home.  Please follow up with you doctor Monday.

Status: offline 		hi all thanks for the replies.  I did pop into the ER   They gave me a tetanus shot- and keflex antibiotics,  I am to get dr check up in a few days as we may need to remove one stitch.   they were not crowded.  rx is closed this hour on Sunday so in the morning.

i am to put heat on it, not ice.

i am glad i went in today.  thank all.  you guys ROCK!!

I agree with almost everything bookworm said, except you can put peroxide on a wound. The peroxide you can buy over the counter is only 3% concentration and not only can you put it on a wound - it wont harm you if you swallow it.  My dentist has me use it - full strength - as an oral rinse before brushing and I accidentally swallowed a mouthful of it. When I called her, she assured me it is ok and also told me that she uses 30% concentration (10 times stronger than what you can buy) Hydrogen Peroxide during oral surgery to stop bleeding. She showed me on the intra oral cam during my last implant - all it does is dry the tissue so the gum stops bleeding and turns white then it goes back to pink in about 10 min. (This is kind of weird but I get along really well with my dentist and she taught me how to do some things - like she showed me exactly how to do the nerve block on the lower jaw and how to infiltrate the anesthethetic for an extraction etc. Obviously not for "do it yourself dentistry" but "just in case" of a disaster situation...) Anyway peroxide is perfectly safe for open wounds, irragating your ears, as an oral rinse etc...

The rest i agree with and one tetracycline wont do anything except weaken the bugs enough that they will survive and come back resistant. For antibiotics to be effective you need the right one for the job and a whole course of them. Thats why in my hurricane survival kit I have 100 ciproflaxins and 100 keflex and a vial of injectable penicillin procaine G...

Absolutely dont remove sutures or staples unless you have been properly trained to and can do it aseptically... That being said, I probably would have sewn it up myself or used crazy glue instead of going to the ER... My kitchen table surgery / suturing used to make my ex faint LOL but our local hospital was a teaching hospital so I wasnt going to spend 5 hours and $1500 to ultimately have a student who doesnt know what hes doing learn something I already know how to do .

Sorry to hear Yr injured, pahunkboy.  It's wise of You to see the MD; such symptoms can indicate that blood poisoning -- which can be fatal -- has begun taking hold.

Status: offline 		Glad you got your ass to the ER and had it relooked at/dressed, Hunk. Do please heed Windchymes advice re. antibiotics. Any old ones, dump, immediately. Never take them unless they are directly prescribed and always use the entire course.

I have been taking the tetracycline, which fell out of the sky- it isnt my rx per unused portion.  [can you say online pharmacy]  anyhow- yesderday I went into black-out mode which is not due to drinking, I dont drink, but is part of having a spinal cord injury. [4 herniated disks, never de-compression, carpal in both hands]  anyhow as soon as the pharmacy opens I will go in. The ER said if it cant heal we will have to break a stitch to let the puss out.  To past month has been hard on the hands to to hurricane weather systems. 

One thing I learned thru these injuries;  never just lie in bed for days on end.   Use it or lose it.  2. dont let them easily operate on the back.

I missed the garbage man.  shoot.  2 beds are not all messed up. I went from upstairs to down. back and forth.

The grass is a jungle.

The parrot needs attention.

oh on family visiting- who knows.  One bro wants to move to FL, the other MO, then sister TX, which leaves mom where??
she isnt one to go long distances.

anyone want some eggs?

Status: offline 		All right. I now have the 500 mg keflex.  I had the pharmacist look at the stitches, he said it looks fine.  He said keflex is a different class the tetracycline.

Try using one hand the whole day.  So I dont have to rush for a recheck... as it is I have to figure out when the psy-dr appt is.  I dont know that I feel like driving for that.

Indian summer today.  I love it. I can use my upstairs.  :-)

I am thinking that I will not embark on any more projects that it requires more then 1 man.   I can count on myself... and that is how it is.   The whole porch enclsure thing- my brother told me not too, and I proceeded anyway as I was tired of the grime from the highway.  Being that my permit is runing out- I finally started taking the windows apart to install them.    I think the neighbor across the road is wrong the wondows need shimmed.  I think they make them differently. Anyhow I really dont want a 2nd chef hanging around.... so well.  :-)

hmm. I was going to ask how do I open one of the stitches.

The hand has a greenish tint to it.  2.5in x 3 in area.   the pharmisist seen it at 9am.it was fine then.    I figure wait to 4sh or 5. I would have had 2 of the 500 mg keflex.

coming from a CMA....DONT EVER PUT PEROXIDE ON A CUT THAT NEEDS STITCHES ESPECIALLY AFTER STITCHES HAVE BEEN PUT IN!!!!!

that would feel like acid in the wound and could actually cause chemical burns to the inner flesh....BAD BAD BAD idea

Stop taking the antibiotics now. What you are doing by not taking the full recommended dosage is building a resistance to the germ (if there are any)

Second...never use Peroxide for anything other then the initial cleaning of the wound. Once healing begins the peroxide will destroy the new tissue growth.

Swelling is normal due to the trauma. But..if it continues to swell or the initial reddness gets worse it could be a sign of infection. Heat is also a HUGE red flag. If these occur haul it into the ER or doctors office asap.

Status: offline 		If you think it's not healing properly, call your doctor.  Not your pharmacist.  And don't take out your stitches while your wound is healing.  Again, let your doctor make that decision and he can do it for you.
