I had not heard of the suggestion before, and I am not without experience in clubs, and have been a DM. If I heard MAYDAY being shouted in a tone to me that suggested real distress, particularly repeatedly, I'd go right over, see what was going on. Just the same as if I heard the particular code word for that dungeon or even a Help! in a 'real' tone.(Any parent should be able to tell the difference between real and 'play' distress!) If it looked really dangerous, eg imminent danger of choking, suffocation, broken bones, hands or feet white or blue through lossof circulation, somebody suspended on kit not designed for the purpose, anybody with eg a single-tail, knife or needles who was not in full control of themselves through eg drink, drugs or anger, I would intervene immediately and answer questions later. Otherwise I'd quietly seek out the DM and suggest he/she checks tactfully to see everything was OK. I hope everybody else with enough experience would do the same. If you don't have the experience, make sure somebody who does is aware.
