We (my Mistress and I) usually play on a extreme level in many ways.  I don't have a safe word with her (though I do have ways to communicate if I feel there is a danger). Six to eight hours (nonstop) bondage with heavy S&M (breath play, lots of needles, clamps and lots more) is not uncommon.
Sometimes in a middle of the extreme scenes comes a unrelated sensation or feeling that's bugs the hell out of me... like when you suddenly have a small stone in one one shoe or there is a trivial part in the bondage that needs to be shifted or adjusted a tiny little bit.  With all these extreme sensations going on, like constant pulling on needles in all possible directions so that even thinking about breathing, which in it self is not always possible,  just plainly hurts...who cares if there's a small stone in the left shoe or not.  I just have to get rid of that stone, no matter what.  It can be funny to watch the enormous relief when the small stone is removed considering all the other extreme sensations at the same time.  If she would put the stone there on purpose... I would not mind it at all.

Have you experienced something similar?

Hmmmmmmmmmmmm...well, I have had a submissive tell me that they often tried to visualize something other than the pain I was dishing out so that they would not slip into subspace faster than they wanted but I don't believe I ever heard her say that she actually felt pain somewhere else during this visualization.

Since my experience in sadomasochism is pretty much zero, particularly compared to yours, even if it was common, I probably wouldn't have encountered it.

But I'd like to make the comment that this seems pretty interesting, from the psycological standpoint. 

For me, personally, the pains that bother me the absolute most are the ones that I can't identify.  Probably contributes pretty strongly to my phobia of needles, since I never know exactly what they're doing inside of my body.  Even headaches, when I can't identity if they're a migrane or sineous, can blow up.  Little itches that might be a splinter after I've worked with wood, even though if I didn't think there was a possibility of a splinter, I would normally hardly even make a passing notice of..

I wonder if your sensation is similar?  If it's because you don't even know if it's actually a stone, or feel that the stone shouldn't be there?  Or any other ideas why it might bug you so much?

I know what you mean, but I don't think I'd ever break the vibe to, say, get my nose scratched or make a tie less pinchy or something.  I just consider it part of the overall suffering and self-denial that I enjoy in its self as well as to please my dominant.

it's a distraction from the intended focus. As you said, if she'd put it there on purpose, it'd not been a problem.

For violet (my sub side), unwanted (by me OR Him) pain becomes a terrible distraction and i WILL interrupt the vibe to get it attended to otherwise it can entirely ruin the play for me. So i have asked Master to change my arm position, scratch my nose, rub a cramped muscle etc ... and He gladly does so because the infliction of that pain WASN'T under His control or desire and He doesn't want it to distract me from that which He IS deliberately inflicting. And as i found out the first time it happened and i didn't ask Him to deal with it and it ruined it for me and thus for Him ... it's not worth leaving it!

It has much less to do with pain and is more of an irritating sensation with or without a physical distractor, which like Vanatru said distracts from the intended focus.

Most of the time I have no problem processing the little "extra" and often trivial sensations that accompanies long term bondage. I think for me it's more when the overall sensation (pain) is on the higher end that the problem arises.  Sometimes it even hinders me in processing the pain that she is administrating.

If she would tell me when the problem arises that she wanted me to endure it, I would have no trouble doing it.  But it could lead to lower pain threshold.

I'm required to tell Himself of any issue that comes up not of his doing (IE hair getting in my mouth is common because of it's length) or which he might not notice right away because of body position or the like. If he told me to keep my trap shut (or I was unaware of his preference) about such things, I would, but this is a specific issue which we've discussed so I know exactly what I'm supposed to do.

Ugh, this is the bane of my bottoming existence.

Nothing more irritating than for a while not being able to focus on or enjoy a good beating because my thigh itches or a piece of hair is tickling my face or some such.

A hair in my mouth stops everything immediately. We don't play that extreme but yes, considering all the other sensations going on, it is odd that one little thing leaps to the top of mind and is all I can focus on until it's removed.

i have asked for my nose to be scratched in the middle of a scene...it becomes part of the scene though..."awww does that feel better..poor wittle ting...poooooor nose..been itching for a long time? oh well" presume wailing away even harder....

There are a few things that could distract, like: hair in mouth, not being able to breathe, coughing fit, handcuffs too tight, cutting off circulation, horrible leg cramp, things like that.

Never had a stone in my shoe though...

Huh, I still don't get it.  If I'm being tormented by someone who doesn't care about my feelings (for the time being), it would be totally bassackwards to the headspace to expect my comfort to be catered to.  Am I the only one?  That's a first!  And here I always made myself feel better thinking there were always other pepople crazy like me out there! What to do with myslef now?

I undestand you point.  It's not always that the there is trouble that needs to be adjusted.  And it can often be fixed by saying "tuff luck" or expressing a will for me to endure it.  Simply asking me if I'm OK in a heavy scene can completely crash my headspace.
