When I was undergoing physical therapy for my broken shoulder, the therapist used a TENS device. He set it at a minimum setting and showed me the knob to turn to increase the intensity, if I felt that I could withstand more. I usually cranked it up high enough so that it felt as if I had shorted out 110v AC with my hands. Being shocked by 110v AC is no big deal for me. I don't make a habit of it but I don't really get too bothered if I run across it. Now, 40,000v from either a car's coil or a CRT television's high voltage circuit is literally a pain in the ass, as well as the extremities and everything else. I've felt both and I would NOT want to repeat that experience again.
