The "hanky code" in BDSM is also something that is used as a sort of merit badge system in some communities. I know it's never really caught on in the het BDSM world, but it's still alive and kicking in the gay male community. To give an example of how pansexual my old home town of Denver, Colorado is, I can tell you that a young lady of my aquaintence spent several months mentoring to a pair of gay leathermen to earn her temp peircing hanky. As I recall, she had to both receive, and give, 100 25ga in rows up and down her back, and her sub's back, before they would let her "graduate" and display the hanky.
Is it disappearing? Is it a myth? No, and no. Like most systems of kinky shorthand communication, it just falls in and out of fashion, and/or mutates into something similar.
When at pansexual events, I usually got better results wearing a t-shirt displaying some image related to what I was into, rather than displaying some kind of "code." Or instance, I made up a t-shirt (using iron on transfers of images I've scanned) of both pony play and of John Willie-style rope bondage. It broke the ice quite easily...
